Understanding Diabetes Management in General Practice

Diabetes is a chronic condition affecting how your body processes blood glucose. Whether you have been diagnosed with Type 1, Type 2, or gestational diabetes, effective management requires ongoing medical support, lifestyle adjustments, and regular monitoring of your blood glucose levels and HbA1c. At Main Street Medical Centre, our GPs take a comprehensive approach to diabetes care. We work with you to develop individualised management strategies, set achievable health goals, and coordinate care with specialists and allied health professionals throughout the Hervey Bay region when needed. Our practice follows current clinical guidance from the Royal Australian College of General Practitioners (RACGP) to support evidence-based, patient-centred care.

Frequently Asked Questions About Diabetes Hervey Bay Care

What is a GP Chronic Condition Management Plan (GPCCMP)?
A GPCCMP is a Medicare-funded care plan for patients with chronic conditions like diabetes. From 1 July 2025, this plan replaced the previous GPMP and TCA system. Your GP prepares the plan to document your health goals, treatment strategies, and coordinate referrals to allied health professionals. Eligible patients may access up to five Medicare-subsidised individual allied health services per calendar year, with referral. Eligible Aboriginal and Torres Strait Islander patients may access up to ten services in total under the relevant arrangements. Your GP confirms your eligibility; allied health providers may charge a gap.
How often should I see my GP for diabetes management?
Many people with diabetes benefit from GP reviews about every three months, however your GP will recommend a schedule based on your individual needs and how well your diabetes is controlled.
What is the National Diabetes Services Scheme (NDSS)?
The NDSS is an Australian Government initiative that provides subsidised diabetes products and support services. Once registered (your GP can help with this), you can access discounted blood glucose testing strips, pen needles, and other eligible items. Registration is free for eligible people.
Do you bulk bill diabetes consultations?
We bulk bill eligible Medicare card holders for many standard GP consultations, including diabetes management appointments, in line with Medicare requirements. Some consultations or additional services may incur a fee — please ask our team when booking.
What allied health professionals can help with my diabetes?
Your GP can coordinate referrals to allied health professionals such as accredited practising dietitians (nutrition), podiatrists (foot care), credentialled diabetes educators, exercise physiologists, and optometrists (eye screening). With an eligible plan, you may be able to access Medicare rebates for certain services.
I had gestational diabetes – do I need ongoing care?
Women who have had gestational diabetes have an increased risk of developing Type 2 diabetes later in life. Guidelines commonly recommend testing 6–12 weeks after delivery, followed by ongoing screening based on your risk factors. Our GPs can support your long-term monitoring and prevention plan.
