GP Management Plans & Team Care Arrangements

If you have a chronic medical condition that has lasted for at least six months, you may be eligible for a GP Management Plan (GPMP) and a Team Care Arrangement (TCA).

A GPMP is a personalised healthcare plan developed by your doctor to help you manage your chronic condition effectively. It includes:

  • ✔ Regular follow-up appointments to track progress.

What is a Team Care Arrangement?

A TCA is designed for patients who require input from multiple healthcare providers. This may include:

  • Specialist doctors for ongoing management.
  • Referral pathways for additional medical support.

Eligible patients may receive Medicare rebates for up to five allied health visits per year under a TCA.
